MLARP Pays $100,000, and Caps You at Two Obligations for Life

A $100,000 programme that is unusually clear about its own limits, including one most state programmes do not state at all.

From the Maryland Department of Health's Maryland Loan Repayment Programs Guidelines, dated 3-13-26 for FY27, a 21-page document read in full:

This is worth setting out carefully, because the guidelines say it four times and not in the same words.

★★ Three of the four read as 40 or more. Only the summary footnote reads as more than 40. We are not going to resolve a state agency's drafting for you, and we are not going to pretend the conflict is not there: if your contract says exactly forty hours, ask the Department where that lands before you plan around an award.
Part-time is 20 to 39 hours per week worked on every wording.
★★ The requirement that catches more people than the hours do
At least 80% of work hours must be dedicated to direct patient care. Certified nurse midwives have their own standard: at least 50% of work hours in normally scheduled clinical hours in an ambulatory care setting.
★ And: "Time spent on-call shall not be counted toward practice hours." For a specialty with heavy call, that is a real constraint on how the week is measured.
★ One piece of flexibility runs the other way. "MLRP participants who work full-time (40 or more hours per week) can be part-time or full-time program participants," while someone working 20–39 hours is automatically a part-time participant. So a full-time clinician can elect part-time participation, at the $50,000 ceiling.
★★ The lifetime cap, which most programmes never state
Verbatim from the guidelines: "Eligible individuals may be awarded up to two (2) MLRP service obligations per lifetime."
★ That is a genuinely useful disclosure, and it is strategy-relevant. A clinician who takes a small early award at one credential level has spent one of two. Someone moving from RN to CRNP to CRNA over a career should know the cap counts obligations, not dollars.
★ The guidelines add the mechanics: a current participant "must be in their second obligation year to be eligible to reapply for a second MLRP obligation," and "two consecutive MLRP obligations cannot overlap." So the second bite is sequential, not stacked.
We are not going to advise you on sequencing that. We are flagging that the cap exists and is published, because a reader who does not know it may use an obligation cheaply.
How it is paid
- Two full payments, one during each service-obligation year.
- Service must be at an approved practice site.
- An applicant who breached a prior federal, state or local service obligation is excluded.
- ★ The obligation starts 1 July following the application cycle, and an employment contract must be in effect by then.
- ★ Eligible debt is loans for study leading to practice as a physician or physician assistant. Loans taken after licensure are not eligible.
★ So the money arrives across the obligation, not at the start. For a mortgage that distinction is the whole point. What an underwriter actually sees.
★ What we are not telling you
The application window. Secondary coverage says the 2026 cycle has closed, and the guidelines we read are dated 3-13-26 for FY27, but we did not confirm the next opening at a primary source. So this site publishes no MLARP cycle dates.
Check with the Maryland Department of Health. We would rather send you to the administrator than print a date that costs you a cycle.
