This is the Maryland market most likely to be bought into by someone arriving from somewhere else: NIH, Walter Reed, the Hopkins and MedStar systems, the Bethesda clinical research corridor.
Md. Code, Tax-Property §13-203(b)(1) tests whether you have "never owned in the State" a residential principal residence. A physician who owned in Virginia, Pennsylvania or anywhere outside Maryland has still never owned in the State.

Maryland county transfer and recordation taxes sit outside §13-203, so the seller-pays direction does not reach them. In these four counties they diverge sharply: Frederick charges no county transfer tax, Charles charges 0.5%, Montgomery varies by property value, and Prince George's charges 1.4%.
